The short answer

Know the systems
Ultherapy (microfocused ultrasound with visualization). The handpiece carries an imaging window, so the provider can see the tissue layers before firing. Energy is delivered as tiny thermal coagulation points at fixed depths — commonly 4.5 mm, 3.0 mm and 1.5 mm. The 4.5 mm transducer reaches the SMAS, the fibromuscular sheet a surgeon tightens during a facelift, which is why this platform is the one usually chosen for a slack jawline or a soft submental angle.
Sofwave. Seven parallel ultrasound beams enter together at roughly 1.5 mm, with a cooling plate protecting the epidermis. There is no imaging step, treatments run shorter, and because the energy stops in the dermis there is far less concern about disturbing facial fat pads. It is the better match for crepey texture and early neck laxity rather than a heavy jaw.
Cartridge HIFU platforms. Ultraformer, Doublo, Sonoqueen and similar devices deliver focused ultrasound through swappable depth cartridges. They are widespread internationally and appear in some US medspas. Without an imaging window, results depend heavily on how methodically the operator maps the face, and cartridge quality varies between grey-market and authorised supply.
Radiofrequency devices such as Thermage and RF microneedling systems such as Morpheus8 are often shelved beside these, but they are not the same category — they pass electrical current through tissue rather than focusing sound waves, and their depth is set by needle length or electrode design instead of a focal point.
What it actually delivers
Expect a firmer jaw contour and a cleaner line under the chin, plus tighter-looking skin on the upper neck. New collagen matures over eight to twelve weeks, so the visible peak lands near the three-month mark and a single course generally holds for about a year before laxity resumes its normal pace.
The honest limits matter more than the upside. Focused ultrasound does not dissolve submental fat, does not shorten a long neck, and cannot remove genuine skin excess. Someone whose jowls hang below the jawline is a surgical candidate being sold a compromise, and they will read the modest result as failure. The strongest candidates are usually in their late thirties to mid fifties with early sagging and reasonably thick skin. In very lean faces, aggressive treatment at the deepest setting can thin the fat compartments and leave the face looking more hollow, not younger.
Choosing a system for your neck
Pick the deep-focus route when the complaint is structural: a softening jawline, a blurred submental angle, banding that only shows on camera. Ask how many lines the provider plans across the lower face and neck, since coverage density drives the result far more than the brand name on the machine.
Pick the mid-dermal route when the complaint is textural, when your pain tolerance is low, or when you can only spare a short appointment. Fine crepey skin over the front of the neck responds well to it, and most patients describe the session as warm rather than sharp. If a clinic offers both, that is usually a sign the provider is matching the device to the layer rather than selling whatever they own.
Aftercare and timeline
Redness and mild swelling are normal for a few hours up to two days. Tenderness when you press along the jaw or neck can linger for two weeks, and small firm welts or patches of numbness sometimes appear and resolve over several weeks without treatment.
Wash and moisturise as usual the same evening, use sunscreen on the treated area daily, and sleep with your head slightly raised the first night if you swell easily. Hold off on strenuous exercise for 24 hours if there is visible puffiness.
