How they actually differ


The distinction is bulk heating versus focused heating. Thermage passes monopolar radiofrequency current through tissue; the tissue's own electrical resistance converts that current into heat across a relatively large volume of dermis and upper subcutaneous fat. The treatment tip cools the surface between pulses, so the skin stays protected while the layer beneath is raised to a temperature that causes existing collagen to contract and triggers remodelling afterwards. Because the heating is diffuse, coverage per pass is wide and the sensation is spread out.


Ulthera does the opposite. Its transducers converge ultrasound energy at a precise depth, creating tiny discrete zones of thermal coagulation while leaving everything above and below untouched. Those points sit in a grid, and at the deepest setting they land in the fibromuscular layer — the same plane a surgeon addresses in a facelift. That is the mechanistic reason Ulthera can claim lift while Thermage claims tightening: only one of them deposits energy in a layer that mechanically supports the face. The imaging window is the second differentiator, letting the provider confirm the tissue under the transducer before firing rather than working blind.


The consequences follow directly. Radiofrequency depends on tissue impedance, so the same settings behave differently across a face with variable fat thickness. Focused ultrasound places energy at a fixed geometric depth regardless of what is above it, which makes it more predictable but also less forgiving — a transducer selected for the wrong tissue thickness delivers into the wrong plane. Neither device targets melanin, so both are appropriate across the full range of skin tones, which is a genuine advantage over light-based tightening.


Time course is similar for both. There is a small immediate contraction, then a quiet period, then collagen remodelling that becomes visible from around eight to twelve weeks and continues improving to about six months. Anyone promising a visible lift the following week is describing swelling.


What each one costs

Who should choose which


Choose Ulthera if the mirror problem is structural: a jaw border that has lost definition, softness under the chin, a heavier upper eyelid from brow descent. You should be in the mild-to-moderate laxity range — skin that still recoils when you lift it — and willing to tolerate a genuinely uncomfortable session. Thin faces need caution, since deep energy over minimal subcutaneous volume can cause unwanted hollowing. Expect one session, judged at three months, repeated in about a year.


Choose Thermage if the mirror problem is quality rather than position: crepey skin on the neck or around the eyes, a slightly slack texture on the cheeks, laxity on the abdomen or knees where large-area coverage matters. It is the far more comfortable of the two, which makes it a reasonable entry point for someone who has never had an energy treatment. Set expectations accordingly — it will not restore a jawline that has already dropped, and hoping it might is the most common reason people report Thermage as disappointing.

Can you have both treatments?

How many sessions does each need?
